Oral nutrition is the provision of a patient’s daily nutritional needs by oral route alone or in combination with parenteral nutrition. The basis of the oral route is the small intestine. The routes of administration are the oral (swallow), naso-gastric (NG), naso-jejunal (NJ), and gastrostomy (GS) routes. The peristalsis of the small intestine permits the movement of all types of nutrients, and, therefore, the absorption of the nutrients by the mucosa of the small intestine is the final step in oral nutrition. Nutrients are absorbed in the small intestine via three different mechanisms: convection by lymphatic flow, paracellular transport across the mucosa, and transcellular transport through the enterocytes. The processes by which nutrients are absorbed include: (1) break-down of the nutrients into smaller molecules, (2) adsorption of the smaller molecules by the mucosa, (3) migration of the adsorbed molecules to the basolateral cell membrane by transcellular transport, and (4) uptake of the molecules by the cells. This review will focus on absorption and metabolism of nutrients and will review the physiologic and metabolic effects of oral nutrition. The principles of nutrition by the oral route are reviewed. Oral nutrients are absorbed into the lymphatic circulation and then delivered to the peripheral tissues. This delivery provides continuous nutrition to the tissues during periods of fasting and after major gastrointestinal surgery.
