One goal of this project is to determine if there is a useful difference in the natural history of synovitis between rheumatoid arthritis (RA) patients with or without extra-articular disease. As there is little difference in synovitis on clinical MRI between patients with or without extra-articular disease, the purpose of this study is to examine the role of synovitis on the progression of structural damage in RA. In the previous report (1992), we found that, at baseline, synovitis was not a predictor of subsequent radiographic change after 4 years of follow-up. In this report (1997), we found that, at baseline, the number of tender and swollen joints, the number of erosions on radiographs, the number of anti-CCP antibody positive patients, and the number of patients with active RF or ACPA were not associated with subsequent radiographic change. The patients with the largest number of erosions were those with the greatest increase in erosions, but there were no significant correlations between the rate of change in erosions and the number of tender, swollen, or healthy joints at any time point. Compared with baseline, there was also no difference between baseline and 4 years in the presence of erosions, osteophytes, joint space narrowing, the mean size of erosions, or the size of joint space narrowing. The same was true for the patients with the greatest radiographic damage at baseline. Thus, the most likely mechanism for synovitis causing radiographic progression is due to loss of function. Another goal of this project is to determine whether different types of synovitis may be more or less important in causing structural damage. There was no relationship between the presence or absence of synovitis on a prior MRI and subsequent radiographic change. Of greater interest, the presence of greater synovitis was associated with a greater decrease in the number of joints with erosions from baseline to 4 years.
